The Registration Process:

*To register you must be a Los Angeles County, San Bernardino County or Orange County resident with a valid doctors recommendation.

     1. Fill out your user information below and click register.

     2. On the second page Fill out the "Membership Agreement Form" which includes your
 recommendation information and click e-sign. (Fields with red borders are required.)

INFORMATION IS CORRECT
ON THE MEMBERSHIP AGREEMENT FORM

Due to the legal nature of the document, we CANNOT edit any mistakes.
Therefore, you will need to fill out the form again if there was an error.

     3. Sit back and browse the current menu while we verify your recommendation with your doctor.
         Depending on your doctor, this step should take no longer than 30 minutes.

     4. After verification, you will receive a confirmation e-mail with the subject "Verification Complete"

     5. Log out of the website then back in to activate your new account privileges and place an order.

     Note: Please have your California ID and ORIGINAL Recommendation letter ready for the driver on your first delivery.
